Each client admitted to Cornerstone will participate in a confidential assessment that is used to obtain information. This assessment provides an interpretive summary, giving us information to develop a Treatment Plan for the client. The purpose of the assessment is to also gear Cornerstone services appropriately based on client needs.
Cornerstone uses a team approach to the provision of services. The client and/or guardian will participate actively in the development of their own Treatment Plan and will receive a copy of the plan.. Goals and objectives will be developed with the client to focus on presenting referral issues and to aid in reducing problematic symptoms.
The Treatment Plan process will be reviewed every 90 days, and at times, dictated by the treatment team.
Treatment Team meetings are held weekly on Thursday’s to staff cases and ensure that services are provided and are beneficial to the client. Clients are welcome to attend treatment team meetings; A week's advance notice is requested to aid in confidentiality of other clients. Clients will only be allowed to address their own particular case.
Confidentiality remains an utmost concern and the privacy of the person served will be respected at all times. HIPAA regulations are implemented and followed. Clients will complete release forms at intake and have the capacity to restrict the release of information if requested.
**E-mails, Cell Phones, Computers and Faxes: It is very important to be aware that computers, e-mail and cell phone communication can be relatively easily accessed by unauthorized people and hence can compromise the privacy and confidentiality of such communication. E-mails, in particular, are vulnerable to such unauthorized access due to the fact that servers have unlimited and direct access to all e-mails that go through them. Additionally, Cornerstone’s e-mails and data on his/her computers are not encrypted. Although we practice preventative measures, it is always a possibility that faxes can be sent erroneously to the wrong address and computers, including laptops. Cornerstone’s computers are equipped with a secure firewall along with virus protection software.
